Non-Continuous Enrollment Verification Form

To remain in compliance with UNC Board of Governors admission policies Appalachian State University is now required to obtain documentation for every gap in a student's education history of four (4) months or more. This form is intended to collect this information in two easy steps.

Step 2: Provide your education/employment history since your last attendance at Appalachian

Please provide detailed information about all of the activities (education, employment, long periods of travel, volunteering, etc.) you have participated in since your last attendance at Appalachian. Before submitting the form please ensure that you have an activity provided for every month during the time period since your last attendance. Be sure to provide all of your plans up until your intended enrollment date at Appalachian. Even if you were unemployed or were not enrolled in school please provide a description of what you were doing during that time period.
